The New Hampshire Partners in Health program supports families of children with a chronic health condition. Partners in Health coordinators help guide, educate, and connect families to resources and financial assistance if needed. They work directly with families and can help you navigate the health system or build capacity to manage the chronic health condition. They can also help arrange for special needs during hospitalization and after discharge, help with school planning, etc.
